Qualitative detection of IgM antibodies to Toxoplasma gondii, rubella virus, cytomegalovirus and herpes simplex virus in serum or plasma.

First Diagnostic™ ToRCH IgM rapid test kit box

Intended use

ToRCH is shorthand for a group of infections that can pass from mother to child during pregnancy or birth: Toxoplasma gondii, rubella virus, cytomegalovirus and herpes simplex virus. The ATFirst™ ToRCH IgM rapid test panel detects IgM antibodies to these four agents in serum or plasma.

It is a qualitative test for healthcare professionals and an aid to clinical assessment. It is not a substitute for reference laboratory serology. First Diagnostic™ Corporation distributes the panel within its ATFirst™ rapid tests range.

How the test works

IgM is the antibody class that generally appears first after an infection. Panels of this type usually pair an anti-human IgM reagent with antigen from each agent: one is fixed at the test line, the other carries the colored label. Each agent has its own test line or strip, and a control line shows that the run is valid.

How the four results are arranged on the device is shown in the insert. See also lateral flow tests explained.

What to confirm in the insert

  • The layout of the panel and which line or strip belongs to which agent
  • Specimen handling and sample volume
  • Read window and detection level
  • Storage conditions
  • Interpretation, including known interfering factors

These points are defined by the insert supplied with each kit. The current insert is available on request from sales@firstdiagnostic.com; the product inserts page lists those already published.

Limitations

  • IgM can persist long after an infection and can reappear, so a reactive line is not proof of a recent first infection.
  • IgM assays are prone to cross-reactivity, and rheumatoid factor can cause false reactive results.
  • A reactive result in pregnancy must be confirmed by a reference laboratory before any clinical decision is taken.
  • A negative result does not exclude infection, for example very soon after exposure.
  • The panel does not detect IgG and gives no information about immunity.
